Your registration will not be processed without full payment. Refunds will only
be given when we receive a notice from a doctor stating that the athlete is physically
unable to participate in the camp. If this occurs a $10 administrative fee will
be deducted from your refund.
You must complete the medical release form and mail to the address
below. A copy of the previous year's physical exam is acceptable to replace physician's
signature.
